The position at Centene is focused on supporting community connection activities that are essential for the care management journey of our members. This role is pivotal in connecting members to community resources that can assist them in receiving necessary care in a cost-effective manner. The individual in this position will provide members with information about known community resources and will work closely with the care team to identify additional support that may be beneficial for the members. Health education will also be a key component of this role, ensuring that members are informed about the resources available to them. In addition to connecting members with resources, the position involves coordinating community outreach efforts to promote awareness of the services and care available to members. The role serves as a support system for members, addressing inquiries related to community and care resources, and facilitating the distribution of educational materials that assist in successful community connections. Documentation and maintenance of community resources are critical responsibilities, ensuring that all practices and policies align with health plan requirements. The position also requires providing assistance to the clinical team, which includes nurses and social workers. This support may involve outreach, community education, and informal guidance to members. The role includes conducting non-clinical general health assessments to refer members to appropriate care and services, resolving concerns on their behalf, and gathering necessary information for medical providers. Non-medical assessments, such as evaluating home safety and community resources, are also part of the responsibilities. The individual may need to conduct telephonic and/or in-person outreach to locate hard-to-reach individuals and families, and may even visit homes or community organizations to provide support. A working knowledge of Social Determinants of Health (SDOH) barriers is essential for this role, as it will help in understanding the challenges faced by members in accessing care and resources. The position may also involve performing other duties as assigned and complying with all relevant policies and standards.
Responsibilities

? Serves as support for members on community and care resource inquiries and opportunities.

? Assists in the facilitation of successful community connections through member-related correspondence and educational materials.

? Documents and maintains all community resources to ensure compliance with health plan requirements.

? Provides assistance to the clinical team of nurses and social workers, including outreach and community education.

? Conducts non-clinical general health assessments to refer members to appropriate care/services.

? Conducts non-medical assessments such as home safety and community resource evaluations.

? Performs telephonic and/or in-person outreach to locate hard-to-reach individuals and families.

? May make visits to individual homes and/or community organizations.

? Demonstrates working knowledge of Social Determinants of Health (SDOH) barriers.

Requirements
? High School diploma or GED required.
