The Population Health Advisor at Blue Cross Blue Shield of Louisiana is responsible for assisting medical groups and affiliate practices in implementing best clinical practices aimed at achieving better health outcomes, lower costs, improved patient experiences, and enhanced clinician experiences. This role focuses on maximizing the use of data to facilitate population health strategies and excel in value-based models, partnering with practices to redesign processes and workflows to improve health outcomes. Success is measured by reductions in total care costs and improvements in quality and efficiency metrics.
Responsibilities
? Assist Medical Group and affiliate practices in implementing population health and quality improvement programs.

? Work with practices to develop an Annual Plan for Performance Improvement.

? Support practice staff in achieving program goals through day-to-day support and coaching.

? Facilitate process and workflow redesign efforts to improve health outcomes.
,
? Represent practices to ensure they have the necessary resources to meet program goals.
,
? Create policy and procedure templates for care coordination integration.
,
? Provide information and resources to practices regarding population health initiatives.
,
? Coach practices on improvement issues and assist with understanding their transformation plans.
,
? Ensure compliance with contractual obligations of Medical Group and affiliate practices.
,
? Communicate data submission requirements to support quality and performance measurement.
Requirements
? Bachelor's degree in Business, Nursing, or a related field required.
,
? 5 years of experience in a dynamic healthcare environment required.
,
? 2 years of experience in financial and healthcare data analysis required.
,
? Experience working closely with physicians and administrative staff in a clinical setting required.
,
? Experience implementing change management processes required.
,
? Proficiency in Microsoft Office software, especially Excel, Word, PowerPoint, and Outlook required.

? Thorough knowledge of ambulatory healthcare and population health models preferred.
Nice-to-haves
? Master's degree in Business Administration or related field preferred.
