Certified Medical Assistant- Chronic Care Management Fast-growing Telehealth provider is looking for an experienced Medical Assistant. Qualifications: ?? CMA or RMA certification (Required) ?? As a Care Manager you would be working with a group of patients over the phone providing teaching, coaching, and motivation to help them improve their chronic conditions. Responsibilities Include: ?? Triaging patient calls ?? Conduct regular condition assessments ?? Patient-Centered Care Plans ?? Monthly med reconciliations ?? Patient Education ?? Contact with providers as needed Qualifications: ?? CMA or RMA License ?? Knowledge of third-party payer requirements ?? Familiarity with electronic medical records ?? Strong aptitude for detail and ability to work independently ?? Strong organizational, problem solving, communication, and interpersonal skills Work Location: ?? Fully Remote This is a 1099 role.
