About the position

We Care Home Health is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide specialized care for an extended care patient with trach, vent, and G-tube needs. This role involves working night shifts in the patient's home, ensuring their safety, comfort, and medical stability. The position offers both part-time and full-time opportunities, allowing for flexibility in scheduling while delivering meaningful, one-on-one patient care.

Responsibilities
?? Delivering specialized care for a patient with trach, vent, and G-tube.

?? Monitoring the patient's respiratory and nutritional health during shifts.

?? Administering medications and tube feedings.

?? Performing routine assessments, interventions, and adjustments as necessary.

?? Communicating with the care team and maintaining accurate patient records.

?? Developing strong, supportive relationships with the patient and their family.

Requirements
?? Current Colorado LPN license.

?? Experience with trach, vent, and G-tube care preferred.

?? Current BLS/CPR certification.
,
?? Strong clinical and communication skills.

?? Ability to work independently in a home care setting.

?? A compassionate, patient-focused approach to nursing.
