Job title: Social Worker Company: Sunnyside Unified School District Job description : DEFINITION: The Social Worker (District-wide) provides intervention and support to families in need of services. Intervenes with students and families in the schools and in the homes. Coordinates with the special education department on placements for students and serves as a case manager for these students. Works (District-wide) independently and must possess a high degree of motivation, self-direction, and accountability. Needs to be flexible in scheduling, as some evening or weekend work may be required in order to work with families. ESSENTIAL FUNCTIONS: (Essential functions, as defined under the Americans with Disabilities Act, may include the following tasks, knowledge, skills and other characteristics. TASKS: Acts as a resource person for the growth and development of students Aids children having difficulty in adapting to school experience and assists when individualized professional help is appropriate Determines and implements appropriate education counseling strategies to effect changes in behavioral ?social interactions of students and their families. Counsels children whose behavior or school progress indicates the need for individual guidance Heads a guidance activity with a small group of students identified as being in need of social services Performs related duties as assigned Assists students, staff, teachers, parents and community members (e.g. applications for services, child and family team meetings, interpreting technical information, etc.) for the purpose of providing and/or conveying information and other services required by parents or teachers. Provides crisis intervention services. Provides social work case management for students and families. Refers students and their families to outside agencies (e.g. state agencies, medical professionals, counselors, foundations, charities, etc.) for the purpose of ensuring the need of students and families are met. Responds to inquiries from a variety of internal and external sources (e.g. parents, students, teachers, staff, outside agencies, etc.) for the purpose of providing information and/or direction as may be required. Communicates with parents on behalf of school (e.g. attendance and homework issues, available programs/services, completing paperwork, etc.) for the purpose of ensuring that an ongoing partnership between the home and school is formed. Confers with teachers, parents and/or appropriate community agency personnel for the purpose of assisting in evaluating student progress and/or implementing student objectives. Coordinates home visits and parent meetings for the purpose of gaining information and/or discussing needs and problems involving students and their families. Prepares and maintains a variety of confidential and non-confidential manual and electronic lists and records (e.g. activity logs, phone logs, community resources, etc.) for the purpose of documenting activities, providing up-to-date reference and audit trail and/or ensuring the availability of items. Participates in a variety of meetings, workshops and committees for the purpose of conveying and/or gathering information required to perform functions and remaining knowledgeable with program guidelines. Visits families and students in their homes as required for the purpose of assessing situations, improving communication, providing information regarding school policies and programs, and/or providing referrals. KNOWLEDGE, SKILLS AND OTHER CHARACTERISTICS: Knowledge of District policies and procedures Knowledge of community based service agencies. Knowledge of services needed for culturally and economically diverse populations. Skill in identifying the needs of individuals and families in the areas of educational, social, mental health, and medical services, then developing and implementing the appropriate services needed. Skill in oral and written communication Skill in establishing and maintaining working relationships PHYSICAL REQUIREMENTS: Work is performed in an indoor environment. Work requires frequent sitting, repetitive motion, vision to monitor, and may require some lifting. QUALIFICATIONS: Master degree in social work or a closely related field Arizona Department of Education School Social Worker certification Three (3) years of experience in social services, mental health settings or a combination of experience, education, and training Dependable mode of transportation Availability to work Flex hours as needed PREFERRED QUALIFICATIONS: Experience with bilingual (Spanish/English) populations desired Depending on the needs of the District, some incumbents in this class must be able to demonstrate fluency in both Spanish and English as a condition of employment.
