Conduct field visits to members in Peoria and Tazewell Counties, IL to assess their needs and develop individualized care plans.
Collaborate with healthcare providers, community resources, and other stakeholders to coordinate and facilitate the delivery of quality care to members.
Utilize case management software to document and track member progress, interventions, and outcomes.
Provide education and support to members and their families on disease management, treatment options, and resources available.
Advocate for and ensure that members receive appropriate and timely medical care and services.
Identify and address any barriers to care, such as social determinants of health, and develop solutions to improve member outcomes.
Monitor and evaluate the effectiveness of care plans and make necessary adjustments as needed.
Communicate and collaborate with interdisciplinary team members to ensure coordinated and comprehensive care for members.
Participate in ongoing training and development opportunities to enhance case management skills and stay updated on industry best practices.
Adhere to all company policies, procedures, and legal regulations to ensure compliance with state and federal laws.
Maintain a high level of professionalism and confidentiality in all interactions with members and stakeholders.
Act as a resource and support for other team members, providing guidance and assistance as needed.
Attend meetings and conferences to stay informed about industry trends and network with other healthcare professionals.
Continuously strive to meet performance goals and improve member satisfaction.
Maintain accurate and timely documentation of all case management activities and outcomes.
