The Care Coach 1 at Humana plays a vital role in assessing and evaluating the needs of members to help them achieve and maintain optimal wellness. This position involves guiding members and their families towards appropriate resources that support their care and wellbeing. The Care Coach 1's work assignments are typically straightforward but can involve moderate complexity, requiring a thoughtful approach to member care. The role necessitates a physical residence in Kentucky or within a reasonable commuting distance to the state, ensuring that the Care Coach can effectively engage with members in the local community. In this position, the Care Coach employs a variety of strategies and techniques to manage health issues faced by members. This includes identifying and resolving barriers that may hinder effective care, ensuring that patients are progressing towards their desired health outcomes. Continuous monitoring of patient care is essential, utilizing assessments, data analysis, conversations with members, and active care planning. The Care Coach must understand professional concepts, standards, regulations, and operating procedures relevant to their work area, making informed decisions regarding their work approach and priorities while adhering to established guidelines. The Care Coach 1 is expected to work remotely, with a structured schedule of 8-hour shifts from Monday to Friday, between 8:00 AM and 5:30 PM. The role involves minimal travel, anticipated at no more than 5%, primarily within a 40-50 mile radius of the market. To ensure effective remote work, employees must have a reliable internet connection and a dedicated workspace that protects member privacy and complies with HIPAA regulations. Additionally, the role requires a valid driver's license and proof of automobile insurance, as it is part of Humana's Driver Safety Program. The Care Coach will also be subject to a Tuberculosis (TB) screening as part of the hiring process.

Requirements
? Bachelor's degree in Social Work, Sociology, Psychology, Gerontology, or a health-related field with at least 2 years of relevant experience, or a Registered Nurse (RN) licensed in Kentucky with at least 2 years of relevant experience, or a Licensed Practical Nurse (LPN) licensed in Kentucky with at least 4 years of relevant experience.

? Ability to use various electronic information applications and software programs.
