About the position

The Telephone Case Manager RN at Optum plays a crucial role in supporting a diverse member population by providing education, advocacy, and connections to necessary resources. Responsibilities
• Make outbound calls and receive inbound calls to assess members' current health status
,
• Make referrals to outside sources
,
• Identify gaps or barriers in treatment plans
,
• Provide patient education to assist with self-management
,
• Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
,
• Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels
,
• Work with less structured, more complex issues
,
• Serve as a resource to others

Requirements
• Undergraduate degree or equivalent experience
,
• Current, unrestricted RN license in the state of employment
,
• 3+ years of experience in a hospital, acute care or direct care setting
,
• Ability to support a Hybrid work schedule, including consumer centric hours (11a - 8p CST) two days a week
,
• Ability to type and navigate a Windows based environment

Nice-to-haves
• BSN

• Certified Case Manager (CCM)

• Case management experience

• Experience or exposure to discharge planning

• Experience in discharge planning or utilization review

• Experience in a telephonic role

• Experience in a leadership, account management or client facing role
